Equifax's internal security policies were a mess and directly led to one of the largest recorded data breaches in history, according to a lawsuit, demonstrating fundamental insecurities inherent in the use of passwords.

Three hit by network outage
A major outage hit the Three communications network at about 11pm on 16 October when the telco suffered a loss of voice, text and data services across the UK because of what it described as “technical difficulties”.
